Arizona radiation therapy prior authorization

The Optum Care Utilization Management team has partnered with Optum’s Cancer Guidance Program (CGP) to process prior authorization requests for outpatient radiation therapy. 

CGP will process prior authorizations for Optum Care UHC members with a cancer diagnosis in AZ.

In general, the following radiation therapy services will require authorization:

• Intensity-modulated radiation therapy (IMRT)
• Proton beam therapy (PBT)
• Stereotactic body radiation therapy (SBRT), including stereotactic radiosurgery (SRS)
• Image-guided radiation therapy (IGRT)
• Special and associated services (e.g. dosimetry and special physics consults) 
• Fractionation for prostate, breast, lung and bone metastasis cancers when using IMRT, PBT or standard 2D/3D radiation therapy 
• Selective internal radiation therapy (SIRT), Yttrium 90 (Y90) and implantable beta-emitting microspheres for treatment of malignant tumors
