Our care and services
We offer primary care at more than 50 locations for family medicine, internal medicine and pediatric care. We provide ongoing care, checkups as well as tests and treatments for minor injuries and illnesses.*
We now provide mental health services at most of our primary care locations. Please call your doctor’s office to learn more.
We can help you with your Medicare Annual Wellness Visits and in-home visits.

Our Extensivist Program offers care for recently hospitalized Medicare Advantage patients for up to 90 days. We help you with chronic (long-term) illnesses and getting better after being in the hospital.
The goal of our program is to help you get better after being in the hospital so you can go back to seeing your primary care doctor.
Hospitalists are doctors who specialize in caring for patients in the hospital. They work with other doctors on your care team.
Hospitalist care is included with our medical care services in the Denver area.
